Quotes cover a standard depth. Each extra foot of shaft adds about $20 to $35 for residential piles (HomeGuide, Forbes Home) and up to $90 per foot for heavy commercial extensions (Techno Metal Post Alaska). The calculator uses $20 to $35 per foot, or $20 to $90 for commercial.
Bringing a machine to site is a fixed cost. One Toronto installer quotes about CAD $500 to $800 (GAMCON); others charge trip fees or set a minimum pile count. The toggle adds $500 to $800.
